How do I find these cab hinges?

These look like a Standard Part to me but I'm not sure what to call them.

Just cable clamps... might find them in the Hillman drawers or McMaster Carr? They do seem to grow legs and walk away. Maybe check with Original Cab as a replacement part?

The metal is too thick to reform on the one bent one we have. 

I'm missing one too though so I figured to purchase two.

Trina can poke around and see what we have.  She's the local gate manufacturer and hinge stock manager.

If your door has 3, steal the middle one and replace it later! Tractor stuff is more important! LOL

Probably a bigger hole on the door hing so you can even bush it too! Smoothness

Give OTC a call. They look like stock door hinges that are on their cabs. Frame work looks like OTC as well.

You can open it up if you need a little more hole, or cut just a sliver and knock it closed more if you need a smaller hole

Weld a piece of tubing to a piece of flat stock.  You could even take a few nuts of the proper size welded together and drilled out then welded to the flat stock.

@JCM  agree with you on the opportunity  , typically those stamped , ends have a  structural crack , right there , adding a front / back  light steel  reinforcement , along with FENDER WASHERS , to spread / reinforce the elastic  lock nuts , would make that a solid point , instead of a flexing / cracking  area .  https://www.google.com/search?q=preforated+steel&rlz=1C1CHBF_enUS866US866&oq=preforated+steel&gs_lcrp=EgZjaHJvbWUyBggAEEUYOTIJCAEQABgKGIA regularly use this for a go to point ,  only a suggestion , also easily trimmed up with tin snips . still have  this in stainless steel . insure a heavier flat washer , to spread the connection . pete
