One I did last week I noticed that one of the head bolt holes at the exhaust port was actually drilled into the exhast port just a little bit.  The bolts came out fine but I noticed the end of the bolt was covered in exhaust soot.  Looking down the hole along one side you could look into the exhaust port.

 

It's always a good idea to 'chase' the threads with a tap before reassembling.

 

I posted this pic in a couple other threads recently, it illustrates why it's a good idea to pull the head every few years to clean the combustion chamber, flatten the head, and renew the gasket.  This one was very close to blowing completely.  This is from a K301.  Note that the location of this is at the inside bend, same place as the other earlier pics showed gasket failure, right near the exhaust valve.
